Some of the available programs include the Community Care Services Program (CCSP) and the Home and Community Based Services Program.

Community Care Service Program

CCSP provides a wide range of community-based services designed to delay or prevent more costly nursing home placement. These services include telephone screenings, in-home assessments, care coordination by an RN or a social worker, home health services, adult day care, alternative living services, out-of-home respite care, personal support services, home delivered meals, and emergency response systems.

Home and Community Based Programs

Home and Community Based Services (HCBS) are in-home and community services designed to assist Individuals 60 and older. Services are designed to assist in keeping seniors healthy, active, and independent in their community as long as possible. Priority is given to individuals who have a low income, those with limited support systems, and those who are considered to be at risk for institutional care due to physical limitations.
