What We Can Do For You At- A -Glance……….

  • Powerful Resource Database with over 23,000 service providers for recreation, in-home care, transportation and many other resources.

  • Community Care Services Program(CCSP) for assistance in/out of  the home with personal care, light housekeeping, meals, daytime care, emergency response devices and 24-hour supervision with the benefits of Medicaid to include consumer-directed care to promote consumer choice and independence, financial management, disease management and customer satisfaction.

  • Service Options Using Resources in Community Environments (SOURCE) to provide elderly, disabled and low-income patients with a wide range of long-term health services, either in their home or community. Services are similar to those provided by CCSP.

  • Home and Community Based Services Program (HCBS) for assistance in the home for persons 60 and older with physical limitations.
  • Connect seniors and persons with disabilities to assistive devices like wheelchair ramps, grab bars and other home modification services.
  • GeorgiaCares to help consumers understand and access Medicare programs, provide information about health insurance options, plan for long-term care and increase awareness of senior fraud
  • Caregiver Support/Kinship Care to help caretakers and grandparents care for their loved ones and grandchildren by offering caregiver services, training and support.
  • Elder Rights and Advocacy to protect the rights of older adults through programs such as Elder Legal Services, GeorgiaCares and Ombudsman services (services to protect residents in long-term care facilities like personal care homes and nursing homes) and to advocate for more service options.
  • Nutrition and Wellness to promote healthy aging through education about nutrition, exercise, disease management and safe usage of medication.

  • Community Capacity Building to help aging service organizations, faith communities and other groups find creative, effective and new ways to meet the needs of the elderly/disabled and their families through presentations, training forums, newsletters and general information
